What would cause the fuel exastion?

The story indicated that when the pilot switched from the aux to the main tank, he didn't get the fuel valve in the proper position, and thus, never opened the main fuel valve back open. Could also be that he didn't use a boost pump. One more reason to always split the times you switch from main to aux and aux to main by at least 5 minutes. I usually split the times by 15 minutes just to be safe.

I have heard there is a pin that can shear on the shaft. Would that be what happened?
